Start searching in the east of Grymforge. After you go down the iron stairs and reach Abandoned Refuge, on the other side of the location you will see a locked door that requires lockpicking skills or a key. After you unlock it, go up the stairs to the top. The item you need will be in the skeleton inventory, loot his body and get the Shield Mould.
Here are the coordinates of the skeleton: X:-565, Y:408.
You can use this item to create the Adamantine Shield. You will also need Mithral Ore for crafting. And you can find it at the top of the stairs that lead to Adamantine Forge. Following the path to the south, jump over the lava to get to the Mithral Vein.
When you collect all the necessary items, you can start crafting. Combine Shield Mould with Mithral Ore and you will get one of six Adamantine items.
