The good news is that getting baleful fragments in Diablo 4 is pretty straightforward. You basically only have to salvage legendary weapons at the blacksmith. Logically, you should not use the weapon you want to upgrade for this. By salvaging, you have a chance to get one or more baleful fragments.
This immediately leads us to the bad news. Because getting legendary weapons in Diablo 4 is not that easy and requires a certain amount of luck. You can usually get the powerful items by completing dungeons, defeating bosses, or running strongholds. Once you have found a legendary item, you must be sure you don't want to use it. You should only salvage it at the blacksmith to hopefully get a baleful fragment.
Tip: Up to level 50, you should only upgrade your equipment to level 3 at the most, and actually only your weapon. You change all other items too often when leveling, and no legendary crafting materials are needed up to level 3.
