In Baldur's Gate 3, players can romance many characters, and the most unexpected of them is The Emperor. After you defeat the Ketheric Thorm and set up camp near Baldur's Gate, your task will be to find and protect the Dream Guardian from the Githyanki invasion. After you meet the Dream Guardian, he will be a Mind Flayer called The Emperor. This is a mysterious character whose story will be hidden throughout the game. To get the Mind Blown achievement, you must be as friendly as possible to The Emperor and accept all his suggestions and ideas. Here's what you need to do to have a romance with The Emperor:
Accept The Emperor's offer and partially transform into an Illithid. By taking the Astral Tadpole, you will receive Illithid Powers in addition to transforming your appearance.

Visit the basement in Elfsong Tavern and find his hideout. Once inside, kill all the rats and move to the western part of the room, where there is a blocked passage. In the next room, defeat one more attacker, go to the statue, and press the button to get to the secret hiding place. Here, you need to interact with all the objects and steal all the items that belong to The Emperor. One of the unique things you'll find in the vault is a mention of his ex-lover, Duke Stelmane. After completing the quest, take a long rest for the scene with The Emperor to take place.
During the long rest, a scene appears in which you have a heart-to-heart with The Emperor in the Astral Prism. Sometimes, this scene does not appear immediately. In this case, take another long rest. During the conversation with The Emperor, ask him about his past relationships until the option to take his hand appears. By choosing this action, you will be on the finish line to getting the achievement.
After having a frank conversation with The Emperor, take a long rest, which you will get into a sex scene.
